I, the signee, hereby grant and authorize the Florence County Museum the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of the tour group to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.
This authorization extends to all languages, media, formats and markets now known or later discovered. This authorization shall continue indefinitely, unless I, the signee, otherwise revoke this authorization in writing. I waive the right to inspect or approve any finished product in which my likeness appears, including written or electronic copy. It is the responsibility of the teachers and/or chaperones to notify the staff of the Florence County Museum in the case that any particular child(ren) is/are not allowed to be photographed per the school’s or organization’s consent forms.
I will be responsible for ensuring that all participating members of my group abide by these guidelines. I will notify the Museum of any changes in date, time, or number of participants at least 2 days prior to the scheduled tour.
I agree and understand that by checking the above Consent and typing this Electronic Signature, that this is the legal equivalent of my manual/handwritten signature.
